Do you know how to get rid of the crazy dialog box?

It's driving me crazy... it's back again....

It's not in the Master Shape...  I see it in the "Persistent Events".  I deleted the CIF from Persistent Events, but it keeps reappearing...  

Any help is greatly appreciated.
