Create Line Connector with Rounded Rectangle at End

I'd still like to know if anyone is aware of something like this that has already been created. More than happy to purchase it.

I would like to create a shape like this that uses the dynamic line connector. How difficult is it to create something like this? The text would need to be able to be left aligned. Is anywhere aware of a shape like this that already exists? I've searched with no luck.
