Customized Toolbar

Whenever I create a "Custom Toolbar", for some reason it disappears after a week.  I have re-created my Customized Toolbar at least 25 times within the last 6 months. 

Enough is enough...

Can someone tell me the reason why my Custom Toolbar keeps vanishing?
