It happens to me all too often. The Reviewing toolbar shows up when I open a Visio document, and I can't get it to go away! It always re-appears when I re-open the document!
Here's the fix:
Kill the Reviewing Toolbar! (http://www.visguy.com/2009/11/09/kill-the-reviewing-toolbar/)
