Visio startup with custom menu

I have a Visio drawing with a macro that creates a custom menu.  I want that macro to run each time I start Visio, automatically, WITHOUT having to manually click on it as the initial template to be loaded.  I have searched for and investigated many threads including
Startup switches
Autoexec macros
templates
startup directory & Add-ins

The last one, Add-ins, looked like a good bet, but it turns out only exe and COM files work there, and I don't know how to create these.

Any advice would be appreciated

I think the recommended route is to get Visual Studio (there's the proof that someone in marketing thought the product was a graphics editor!  ;D ) and there are various subsets of this to help you create C++ and C# programs, one option being a Visio add-in.

However, I'm going to suggest you get Delphi, the older versions can be downloaded free, they still work, and then get Graham Wideman's Visio framework for Delphi:

There's another Visio controller framework as well but I cannot find its URL off-hand.
